Senior Practitioner - Reablement Team

Shropshire Council – Reablement Team
Pay rate: £32 Per Hour 
 
Duties Include:

• Support Team manager as required
• Supervision for Social Workers and social care practitioners
• Support with duty response
• Support with managing referrals
• Assessing mental capacity and best interests
• Supporting with community reablement
