Pay rate, £30 per hour
Vitalis are recruiting for an experienced Social Worker to work in a Hospital Discharge Team in Warrington.
2 agency Social Work posts are available within the Transfer of Care Hub (Hospital Discharge Team) at Warrington General Hospital. This post is predominantly based on-site with some hybrid working available. 
The successful candidate will be required to have access to their own vehicle as some visits to out-of-area hospitals are required. 
The ideal candidate will have experience in hospital discharge work or working within an integrated team.
