Complex Discharge Assessments for D2RA Pathway 3, knowledge and skills in Mental Capacity Act and Continuing Health Care Funding required. Agile work between home and hospital sites. Full time only. Car driver essential. Must be registered with SCW. 
Pay - £38.43 per hour  
There is plenty of hybrid working available.
