Adult Social Worker, Hospital Discharge, Islington  
Pay rate £32 per hour 
This will be a short term appointment to cover an acting up arrangement, until longer term organisation plans can be achieved. The role supports discharge planning of Islington Residents at University College of London Hospital and other Out of Borough Hospitals. Experience of Hospital Social Work would be desirable, but critical would be great assessment skills and the ability to work in partnership. There is an expectation to work on site at UCLH and / or @ 222 Upper Street, Islington at least 3 days a week. There is opportunity to work remotely but this will be based on evidence of performance / practice.
