Adult Social Worker, Lambeth  
Pay rate £32 per hour 
To support with home care mobilization project in other to transfer service users to their neighbourhood provide, review of Care needs for Adult . The role with involve speaking with service user and family regarding the change in their care provider and reassuring them. It is Hybrid working however candidate is expected to be present in the office everyday-Monday-Friday for the first four weeks and afterwards, they are expected to attend the office twice a week.
